Self-assessment had produced zero failures for years, leading the institution to conclude that a costly, disruptive audit discipline was no longer needed. Pleus made and won the counter-argument: zero failures under self-assessment isn't evidence of readiness — it's evidence the assessment has stopped testing anything. He reinstated the discipline. The next cycle produced failures, which was the point: the organization could once again see what it was tolerating.
